Terminations Report — Column Reference

Column	What it shows
Name	Worker's full display name
Username	Worker's login username
WorkerID	SimpleVMS internal ID for the worker
ClientWorkerID	Your organization's identifier for the worker
Badge	Secondary client-assigned worker identifier (ClientWorkerID2)
EmailAddress	Worker's primary email address
AlternateEmailAddresss	Worker's alternate email address

Column	What it shows
Site	Name of the site the worker was assigned to
Vendor	Staffing vendor who supplied this worker
Title	Worker's job title
Department	Department the worker was assigned to
CostCenter	Worker's primary cost center
BillableRate	Worker's bill rate at time of termination
Status	Final assignment status (e.g., Inactive, Terminated)
StartDate	Date the assignment began
EndDate	Date the assignment ended

Column	What it shows
Category	High-level category for why the assignment ended (e.g., Voluntary, Involuntary, Contract End)
EndReason	Detailed reason the assignment ended
History	Summary of the most recent end reason change — shows the value it was changed to, when, and by whom
StatusModifiedDate	Date and time the worker's status was last changed to its current value
StatusChangedBy	Username of the user who made the last status change
